Job Summary:

We are seeking a highly skilled and dedicated LCSW to join our team of healthcare professionals. As a Licensed Clinical Social Worker, you will provide individual and group therapy sessions to patients with a variety of mental health diagnoses. Your expertise in social work principles and practices will be utilized to develop and implement individualized treatment plans, assess patient progress, and collaborate with other healthcare professionals to ensure comprehensive care.

Responsibilities:

  • Conduct individual and group therapy sessions with patients, addressing various mental health concerns such as anxiety, depression, and trauma

  • Develop and implement individualized treatment plans, taking into account patient goals, needs, and progress

  • Assess patient progress and adjust treatment plans as needed

  • Collaborate with multidisciplinary teams, including physicians, nurses, and therapists, to ensure comprehensive care

  • Provide crisis intervention and emergency services as needed

  • Maintain accurate and timely records of patient sessions and treatment plans

Requirements:

  • Licensed Clinical Social Worker (LCSW) in the state of CT

  • Master's degree in social work (MSW) or related field

  • 2 years of experience in a Long-Term Care Facility
