The APRN – Primary Care who is either licensed as Family practice FNP or Adult-Gerontology AGNP working with an adult population of age 18 and up in an Rural Health Center (RHC), collaboratively with physicians, providers, and clinical staff to deliver high-quality, patient-centered care to an assigned patient panel. This role includes comprehensive assessment, diagnosis, treatment planning, and ongoing management of adult patient over their lifespan within the scope of APRN licensure and medical staff privileges.

The APRN plays a vital role in promoting preventive care, managing chronic conditions, educating patients and families, and contributing to a positive, team-based clinic environment.

Essential Duties & Responsibilities
  • Manage the care of patients within an assigned panel and scope of medical staff privileges
  • Perform comprehensive health histories, physical exams, and clinical assessments
  • Order, perform, and interpret diagnostic, screening, and laboratory tests
  • Diagnose conditions and develop individualized treatment plans
  • Prescribe pharmacologic and non-pharmacologic therapies
  • Provide patient and family education to support treatment goals and wellness
  • Collaborate with physicians, advanced practice providers, and interdisciplinary teams
  • Coordinate referrals and consultations as appropriate
  • Document patient care, outcomes, and treatment effectiveness in the medical record
  • Reassess and modify treatment plans based on patient response and clinical findings
  • Assist with care coordination, follow-up planning, and necessary documentation
  • Participate in provider meetings and provide cross-coverage within the clinic when needed
  • Support educational experiences for students and learners as applicable
  • Perform other related duties and participate in special projects as assigned
