    • Summary:

      We are seeking a compassionate, organized Hybrid Behavioral Health Care Coordinator to support Allegro's integrated behavioral health program.

      At Allegro Pediatrics, we partner with families to heal, nurture, and empower each child on their journey to adulthood. As the mental health needs of children and teens continue to grow, we have expanded our integrated behavioral health program to provide more coordinated and accessible support for patients and families.

      This role plays a key part in helping families access behavioral health support within Allegro's medical home and connect with appropriate community resources. If you enjoy helping people, building meaningful relationships, staying organized, and making a difference in the lives of children and families, we'd love to hear from you.

      Primary Responsibilities:

      Welcome referrals from pediatricians into the behavioral health program and provide a steady, supportive point of contact for patients and families

      Support informed consent and help families understand the program, next steps, and their role in follow-through

      Coordinate intake, scheduling, referrals, follow-up, and communication via phone and/or email to support integrated care within the child's medical home

      Maintain patient registries, track outreach and enrollment, and keep documentation timely and accurate

      Identify urgent behavioral health concerns and support triage through established workflows

      Guide families to appropriate community behavioral health resources and support warm handoffs when available

      Maintain referral resources and strengthen community partnerships that support access to care

      Contribute to quality improvement by identifying workflow gaps and supporting program refinement

      Experience with an EHR system and willingness to learn Genesys (phones), ProviderFlow (eFax), Phreesia (patient engagement), and Greenway Intergy (EMR)

      Ability to learn new technical programs quickly

      Familiarity with Dropbox preferred

      Understanding of HIPAA

      Requirements:

      Enjoy working in a fast-paced, patient-focused environment with ongoing changes

      Must have reliable transportation and be willing to travel to all clinics as staffing needs change

      Must be 18 years of age or older

      To uphold safety for all employees and patients, we continue to require vaccination status for all new hires, including remote or hybrid schedules, at time of hire and annually:

      o Various other immunization requirements that we can discuss during your interview
      o All employees are currently required to wear a surgical, KN95, or N95 mask at all times while in clinic and patient-facing

      Education:

      High School diploma/GED or higher

      Additional coursework or experience in healthcare, behavioral health, child development, psychology, social services, or care coordination preferred

      Schedule:

      25-30 hours per week, Monday through Friday

      Approximately 5-6 hours per day between 9:00 AM and 3:00 PM

      Following initial in-office training, this role will transition to a hybrid schedule
