Job Summary

BrightCare HomeCare is seeking a compassionate and detail-oriented Care Manager to join our team in Baton Rouge, LA. As a Care Manager, you will play a vital role in ensuring the highest level of care for our clients, providing personalized support and supervision to help them achieve optimum health and wellness. Responsibilities

  • Develop and implement personalized care plans for clients, taking into account their unique needs and health requirements

  • Coordinate and supervise the delivery of care services, ensuring that clients receive high-quality, compassionate care

  • Maintain accurate records and documents related to client care, including intake forms, care plans, and progress reports

  • Communicate effectively with clients, family members, and healthcare professionals to ensure seamless care coordination

  • Collaborate with our interdisciplinary team to identify and address any issues or concerns related to client care

  • Provide emotional support and empathy to clients, promoting independence, autonomy, and overall well-being

  • Participate in ongoing training and professional development to stay current with industry best practices and regulations

  • Participate in recruiting and training of new hires.

  • Participate in scheduling and on call

Requirements

  • 5+ years of experience in a similar care management or healthcare role

  • Excellent communication, problem-solving, and customer service skills

  • Ability to self-motivate and work independently, with a strong sense of responsibility and accountability

  • Strong analytical and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
