Hospice of North Alabama is seeking a licensed LPN or RN with strong clinical and communication skills to join our team as a Hospice Intake Coordinator in Decatur, AL. This is a critical role that helps patients and families transition smoothly into hospice care by coordinating the intake and admission process from start to finish. As an Intake Coordinator You will:

  • Educate patients and families on hospice services, answering questions and easing concerns.

  • Communicate with physicians, discharge planners, and other healthcare professionals to ensure continuity of care.
  • Ensure all regulatory and compliance steps are followed throughout the intake process.
