Description

The Program Support Specialist of Family Connects New Jersey (FCNJ) Hunterdon county provides essential administrative, logistical, and operational support to ensure the successful delivery of the organization's program. This role works closely with program, administrators, supervisors, staff, and external partners to coordinate activities, maintain accurate records, and support program evaluation and reporting. The Program Support Specialist plays a key role in maintaining smooth day-to-day operations, assisting with communications, and ensuring that program participants and stakeholders receive timely and effective support.


Essential Functions

  • Conduct outreach and scheduling of appointments with families through collaboration with community-based programs at PMCH, Connecting NJ, and local birthing facilities
  • Participate in bedside recruitment at hospitals, clinics, and/or birthing facilities
  • Assist with mentoring, precepting, and conducting shadow visits for new PSS staff
  • Complete appointments and patient follow-up call data for home visits using MS Office Suite and Salesforce
  • Document outreach attempts with families using web-based data systems
  • Monitor FCNJ email and phone accounts to respond to inquiries
  • Conduct post-visit calls with clients served to assess services and additional referral needs
  • Participate in weekly team meetings with nursing staff
  • Develop and maintain community/provider referral sources, along with nursing staff

Requirements

  • Bachelor's Degree in Public Health or related field preferred
  • Minimum 3 years of experience working in community outreach, public health, or healthcare
  • Experience working with diverse populations, communities, and organizations
  • Bilingual preferred (Haitian Creole, Spanish, Arabic, Korean, or Portuguese)
  • Excellent verbal, written and interpersonal communication skills
  • Proficient with Microsoft Office Suite
