Value-based Primary Care Physician

Value-based primary care physician role to manage chronic conditions of high-risk geriatric patients Monday-Friday (no weekends or evenings). Spend 30-45 minutes with your patients to manage all chronic conditions alongside a supportive team of providers and office operations staff.

Responsibilities

The physician delivers primary health care and patient care management in a residential setting.

Qualifications
  • Experience in Geriatrics or complex patients preferred
  • Active/unrestricted medical license and DEA
  • Board Certified or Board Eligible
  • EMR experience
  • Must maintain a valid driver's license and good driving record
