Job description

  • Position Summary
    • Under the supervision of the Pharmacy Manager and under direct supervision of Pharmacists, this position is responsible for, but not limited, to providing specialized expertise in obtaining medication histories. The Medication History Pharmacy Intern shall obtain and document accurate medication histories for high acuity patients and all patients being admitted to the hospital as time allows, communicate to the clinical pharmacist any discrepancies found on medication histories that have already been reconciled by the admitting physicians to address any changes needed to inpatient orders, and expedite medications to the bedside by increasing coordination with central pharmacy. Obtain and document allergies, including allergy reaction, for all patients being admitted to the hospital as time allows. Obtain and document patient’s preferred outpatient pharmacy.  Provide medication history training to new staff as assigned by manager. Begin to develop leadership skills and serve as a role model to pharmacy students. Pharmacy interns perform assigned duties independently within the department policies and procedures but never perform duties which can only be performed by a pharmacist.  Pharmacy interns perform assignments within established time frames and work routines may vary according to patient care priorities.

  • Core Job Functions

During low volume times in the ED, assists with obtaining medication histories for patients that were admitted to the hospital during the hours the pharmacy technician is not present in the ED.
    • Communicates to the clinical pharmacist any discrepancies found on medication histories that have already been reconciled by the admitting physicians to address any changes needed to inpatient orders.
    • Obtains and documents allergies, including allergy reaction, for high acuity and all patients being admitted to the hospital as time allows. Obtains and documents patient’s preferred outpatient pharmacy.
    • Provides medication history training to new staff hires as well as pharmacy students.
