RESUME DU POSTE
L'infirmier(ere) charge(e) de la gestion des cas (ICGC) jouera un role essentiel dans le soutien aux patient(e)s, en fournissant un soutien au remboursement aux medecins, ainsi que des services de gestion des cas medicaux aux patient(e)s. Aide le (la) medecin a remplir tous les documents specifiques au programme et assure le suivi des demandes relatives a l'obtention d'un remboursement ou d'une couverture du medicament par des formulaires de couverture privee et provinciale, afin de s'assurer que les patient(e)s aient acces en temps opportun aux traitements qui leur ont ete prescrits. Sert de ressource aux medecins qui prescrivent ce medicament et offre un soutien continu aux patient(e)s pour le suivi, les renouvellements et les changements de couverture.
FONCTIONS ET RESPONSABILITES
Aider a l'inscription des patient(e)s, notamment en recueillant tous les renseignements pertinents et appropries pour remplir les formulaires necessaires (assurance privee et/ou provinciale) de facon concise, organisee et professionnelle.
Assurer la liaison et plaider aupres des fournisseurs tiers et des gouvernements provinciaux, au besoin, pour determiner les options de couverture.
S'assurer que les patient(e)s ont rendez-vous pour les examens diagnostiques necessaires et informer les medecins de tout resultat d'examen perime necessitant un suivi.
Accomplir les activites d'appel de bienvenue avec le (la) patient(e) selon des scenarios 
definis.
Fournir aux patient(e)s et aux medecins un numero sans frais direct. 
Examiner les rapports apres la visite pour confirmer que le prochain rendez-vous est programme, et verifier l'integrite des donnees du rapport par rapport a l'ordonnance du (de la) medecin, le cas echeant. 
Trouver les rapports apres la visite manquants, le cas echeant. 
Faciliter toute communication requise avec les medecins, les programmes de soutien aux patient(e)s ou les autres professionnel(le)s de la sante implique(e)s dans le cercle de soins du (de la) patient(e).
Assurer le suivi et la defense des interets jusqu'a ce qu'une decision soit prise, idealement en obtenant une couverture pour le (la) patient(e), le cas echeant. 
Travailler en etroite collaboration avec les representants commerciaux locaux du fabricant pour repondre aux besoins specifiques des medecins et du personnel, le cas echeant.
Proposer et/ou dispenser une formation aux patient(e)s et aux medecins sur le(s) produit(s).
Agir a titre de personne-ressource centrale pour la liaison avec les parties prenantes du programme, notamment les medecins, les infirmier(ere)s, les patient(e)s et les cliniques.
Coordonner et planifier les visites des patient(e)s dans les lieux appropries (domicile, clinique, etc.), le cas echeant.
Fournir des mises a jour aux parties prenantes internes et externes sur la base des observations.
Veiller a ce que les dossiers des patient(e)s soient tenus a jour et que les donnees soient exactes afin de repondre aux exigences du programme en matiere de rapports.
Assurer une declaration rapide et conforme des evenements indesirables (EI) et des reclamations qualite produit (PQC) conformement aux exigences et directives de pharmacovigilance de Sante Canada, incluant une documentation precise, une escalade appropriee et une collaboration efficace avec les parties prenantes internes et les equipes reglementaires.
Participer aux activites de qualite et aux initiatives d'amelioration continue conformement au systeme de gestion de la qualite de l'entreprise ainsi qu'aux indicateurs de performance specifiques au projet.
Preserver la confidentialite des renseignements relatifs aux partenaires pharmaceutiques et a l'entreprise, et n'en discuter qu'avec le personnel approprie de Bayshore.
Remplir tous les rapports pertinents (feuilles de temps, depenses, kilometrage, rapports, etc.) dans les delais impartis et conformement a la politique de Bayshore, le cas echeant.
Remplir les rapports sur les plaintes relatives aux produits.
Adherer aux politiques et aux procedures de Bayshore et du fabricant.

JOB SUMMARY
The Nurse Case Manager (NCM) will play an essential role in supporting patients with reimbursement support to physicians and medical case management services to patients. Assist the physician with the completion of all programspecific paperwork and followup on submissions pertaining to obtaining drug reimbursement/coverage through private and provincial drug formularies to ensure patients have access to their prescribed treatments in a timely manner. Act as a resource to physicians who are prescribing this medication and continuous support for patients for ongoing support, renewals, and coverage changes.
DUTIES AND RESPONSIBILITIES
Assist in completing patient enrolment; including gathering all pertinent and appropriate information to complete the necessary forms (private and/or provincial insurance) in a concise, organized, and professional manner.
Liaise and advocate with third-party providers and provincial governments as necessary to determine coverage options.
Ensure patients are scheduled for necessary diagnostic tests and notify physicians of any expired test results that require follow-ups.
Complete Welcome Call activities with the patient according to defined scripts.
Provide patients and physicians with direct toll-free number. 
Review post visit reports to confirm next appointment is scheduled and data integrity of the report against the physician order, if applicable. 
Track missing post visit reports, if applicable. 
Facilitate any required communication with physicians, Patient Support Programs, or other health care professionals involved in the patient's circle of care.
Follow up and advocate until a decision is received, ideally securing coverage for the patient, if applicable. 
Work closely with the manufacturer local sales representatives to support specific physician's and staff needs, if applicable.
Offer and/or provide educational training to patients and physicians on product(s).
Act as a central resource liaison for program stakeholders, which include physicians, nurses, patients, and clinics.
Coordinate and schedule patients at appropriate visit locations (home, clinic, etc.), if applicable.
Provide updates to internal and external stakeholders based on observations.
Ensure patient records are maintained and data-accurate to meet program reporting requirements.
Ensure timely and compliant reporting of Adverse Events (AEs) and Product Quality Complaints (PQCs) in accordance with Health Canada pharmacovigilance requirements and guidelines, including accurate documentation, appropriate escalation, and effective collaboration with internal stakeholders and regulatory teams.
Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System as well as specific performance indicators for the project.
Maintain confidentiality of pharmaceutical partner(s) and corporate information and discuss same only with appropriate Bayshore personnel.
Complete all relevant reports (time sheets, expenses, mileage, reports, etc.) as per specified timelines and Bayshore's policy, if applicable.
Complete product complaint reports.
Adhere to Bayshore and manufacturer Policies and Procedures.
Complete other tasks, as requested.
 

QUALIFICATIONS
Formation
Diplome(e) d'un Programme d'infirmier(ere) autorise(e) ou d'un Programme d'infirmier(ere) auxiliaire autorise(e)/immatricule(e), titulaire d'une autorisation d'exercer en vigueur delivree par un organisme provincial de reglementation au Canada.
Experience
Inscrit(e) et en regle aupres de l'organisme provincial de reglementation.
Au moins deux ans d'experience dans des Programmes de soutien aux patient(e)s ou l'equivalent. 
Expertise demontree dans la prestation d'un service a la clientele exceptionnel et dans l'etablissement de relations positives avec les parties prenantes internes et externes. 
Maitrise de la communication verbale et ecrite et des competences interpersonnelles en anglais et en francais (pour les provinces/regions ayant des exigences de service en francais).
La comprehension des criteres de couverture provinciaux et prives, des processus d'autorisation speciale et de la facon de s'orienter dans le paysage de la couverture des medicaments au Canada est un atout.
Competences informatiques et techniques solides : Excel, Word, Outlook, tablettes electroniques, portails Web/utilisation d'Internet.

QUALIFICATIONS
Education
Graduate of Registered Nursing Program or Registered/Licensed Practical Nursing program holding current registration from a provincial licensing body in Canada.
Experience
Registered and in good standing with their provincial regulatory body.
Minimum 2 years' experience in Patient Support Programs or equivalent. 
Demonstrated expertise in delivering exceptional customer service and fostering positive relationships with internal and external stakeholders. 
Must be fluent in verbal and written communication and interpersonal skills for both English and French (Applicable for provinces/regions with French language service requirements).
Understanding of provincial and private coverage criteria, Special Authorization processes and how to navigate the Canadian drug coverage landscape is an asset.
