Summary :
The Home Health Case Manager (Remote) plays a crucial role in collaborating with health care team, patients/families and providers to assist with the plan of care and goal achievements. Responsible to coordinate and ensure care is safe, timely, effective, efficient, equitable, and patient-centered by performing at the top of the RN licensure. Furthers the professional practice of nursing at ThedaCare by promoting a culture of innovation and commitment to growth and professional development. The Remote Home Case Manager will be expected to train onsite and attend meetings, trainings and other events onsite at ThedaCare at Home as needed throughout the calendar year.
Job Description:
Key Accountabilities
  • Coordinate patient care from a distance, primarily using phone or video communication, focusing on assessments, developing care plans, and ensuring patients access necessary resources and support
  • Identify process improvement through data collection and auditing
  • Skilled in navigating multiple EHR platforms and other applications
  • Exhibits clinical excellence, strong communication and ensures the highest quality of care. Use home health expertise to triage patient needs and maintain open communication with patients, caregivers and other healthcare providers. Follow standardized triage protocols to ensure consistency in patient evaluation and delivery of care
  • Manages holistic picture of individual/family/caregiver to promote overall health and wellbeing with focus on patient centered care
  • Facilitates health optimization by progressing the plan of care from admission to discharge by determining the appropriate utilization of services
  • Responsible for the collaboration between the interdisciplinary care team by identifying goals and coordinating the plan to ensure patient safety
  • Creates engagement by taking responsibility for their professional development and other team members as evidenced by leading case conferences, assisting team member in-services, acting as a clinical resource and staff meeting input
  • Demonstrate strong understanding of home health regulations, standards and OASIS RN clinic

Qualifications
  • Education - Bachelors of Science (BSN)
  • Experience - minimum of 3 years home health bedside, EPIC, CIC, Remote Client, SHP, Rover experience preferred
  • Licensure - Current Wisconsin RN license
  • Certifications - American Heart Association Healthcare Provider Basic Life Support (BLS), OASIS Certification preferred

Physical Demands
  • Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of fifty (50) pounds without assistance
  • Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

Work Environment
  • Normally works in climate-controlled office environment
  • Frequent sitting with movement throughout office space
  • Use of computers throughout work day
  • Frequent use of keyboard with repetitive motion of hands, wrist and fingers
  • Frequent exposure to sharp objects and instruments
  • Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock.
  • Occasional high noise level in work environment.
  • Possible exposure to communicable diseases, hazardous materials, and pharmacological agents.
  • Occasional contact with aggressive and or combative patients
