Job description

**This position is based in Wheeling, WV and it is not a remote position. Possible Hybrid**

The Health Coach is a professional that reports to the Manager of Clinical Wellness. They work with members telephonically and/or face-to-face to assess members and enable them to make long-term positive health changes via good advice, motivation, and support. They utilize The Health Plan's wellness library and other relevant health information to educate, inform, and advise members and help them adhere to appropriate wellness guidelines and standards of care.

Required:

  1. Bachelor's degree in public health, exercise physiology, nursing, dietary, health education, or a health related field.
  2. Excellent knowledge in health, nutrition, and fitness.
  3. Knowledge of health related disease conditions and remedial measures.
  4. Excellent verbal and written communication skills.
  5. Strong motivational and convincing power.
  6. Ability to counsel members of all ages and problems.
  7. Solid experience in a variety of clinical techniques and the medical field.
  8. Immense ability to develop a health plan.
  9. Possess a well organized and pleasing personality.
  10. Knowledge of the latest trends in health care.

Desired:

  1. Certification in Health Coaching from The National Society of Health Coaches (NSHC), The American Council on Exercise (ACE), The Well coaches School of Coaching, The Duke Integrative Health Coach Program, Dr. Sears Wellness Institute or Health Coach Institute or another pre-approved program.

Responsibilities:

  1. Provide effective solutions and suggestions on health and fitness issues.
  2. Work closely with the Manager of Clinical Wellness and Director of Clinical Programs in meeting the organization's health management goals.
  3. Provide telephonic and/or face-to-face support and education to members of The Health Plan (THP).
  4. Establish proficiency in THP programs and applications necessary to perform the functions of a Health Coach, including, but not limited to, the care management platform, HEART, Healthwise, and any other relevant program(s)/software.
  5. Demonstrate basic understanding of the Healthcare Effectiveness Data and Information Set (HEDIS, Medicare Stars, Consumer Assessment of Healthcare Providers and Systems (CAHPS), and Health Outcomes Survey (HOS) measures and information to support gap closure and promote preventive screening measures.
  6. Counsel and advise members to improve overall health condition.
  7. Understand the prevailing health conditions and support diagnoses and care for the required problems.
  8. Identify the need for behavioral change to improve health status, reduce health risks, and improve quality of life.
  9. Offer possible solutions to address health related issues.
  10. Obtain health records or record limited health information, such as vital signs or symptomatology to refer to relevant THP programs.
  11. Involve member in motivational interviewing and engagement strategies.
  12. Apply knowledge of medicine, diet, nutrition, and/or relevant methods to suggest solutions.
  13. Suggest exercises, diet charts, meditation, or mental stress reducing techniques to members.
  14. Interact with the member with a positive and empathetic attitude to make them feel comfortable.
  15. 100% of regular tasks are dedicated to care coordination of THP members.
