Position Summary
The Health Educator / Health Coach partners with clients, members, and internal teams to design, deliver, and support engaging health coaching programs. This role focuses on building trusted relationships with members to support sustainable behavior change and improved health outcomes using evidence-based coaching strategies. The ideal candidate is empathetic, adaptable, and comfortable working both independently and collaboratively in dynamic worksite and virtual environments.

Key Responsibilities

  • Proactively engage eligible program participants and support enrollment in wellness and coaching programs
  • Provide one-on-one health coaching sessions to help participants reduce or eliminate high-risk behaviors and improve overall health
  • Motivate and support participants in adopting healthy habits that enhance quality of life
  • Develop personalized care plans, including SMART goals and actionable steps, tailored to individual participant needs
  • Collaborate with participants managing chronic conditions to develop individualized action plans, reinforce treatment adherence, and promote sustainable lifestyle changes
  • Monitor participant progress and conduct follow-up outreach to encourage ongoing engagement, accountability, and successful achievement of health and wellness goals

Qualifications & Skills

Required

  • Bachelor’s degree in a health-related field (e.g., health education, public health, exercise science, nutrition, nursing, or related discipline)
  • At least 1 year of experience in health coaching, health education, wellness programming, or care management
  • Proficiency with Microsoft Office (Word, Excel, PowerPoint) and comfort using digital tools and platforms

Preferred

  • Bilingual (Spanish/English)
  • Experience delivering worksite or employer-based wellness programs
  • Training or experience in behavior change methodologies such as Motivational Interviewing or Cognitive Behavioral Therapy
  • Experience supporting wellness initiatives related to weight management, stress management, physical activity, nutrition, nicotine cessation, or lifestyle behavior change
  • Master’s degree in health-related field (e.g., health education, public health, exercise science, nutrition, nursing, or related discipline)
