About the Role

The Transgender Health and Wellness Program Coordinator is a leadership role that blends program coordination, direct service, and community engagement.

You will oversee the day-to-day operations of the program, support and guide staff, build trusted community relationships, and ensure services are delivered with excellence, compliance, and care.

This role is ideal for someone who is community-rooted, detail-oriented, and passionate about health equity.


What You’ll Lead and Drive

Program Coordination & Service Delivery

  • Coordinate HIV, STI, and HCV prevention, testing, and linkage-to-care services
  • Implement and facilitate evidence-based and community-driven interventions
  • Provide direct services including risk assessments, case management, and patient navigation
  • Ensure all services are trauma-informed, culturally responsive, and rooted in harm reduction

Community Engagement & Outreach

  • Lead outreach efforts that build trust, visibility, and connection within TGNCNB communities
  • Plan and facilitate community events and wellness-focused activations
  • Develop partnerships with healthcare providers, community organizations, and coalitions

Data, Compliance & Quality

  • Ensure timely and accurate reporting to funders and regulatory agencies
  • Maintain compliance with HIPAA and NYS HIV confidentiality laws
  • Monitor program data and conduct quality assurance reviews

Team Support & Coordination

  • Support, guide, and help supervise program staff
  • Provide onboarding, coaching, and ongoing support
  • Foster a team culture rooted in respect, accountability, and care

Operations & Organizational Support

  • Prepare monthly reports and program updates
  • Collaborate across departments to strengthen services and outcomes
  • Support organizational priorities and program growth


What You Bring

Required

  • Bachelor’s degree in Public Health, Social Work, Human Services, or related field
  • 2+ years of experience in HIV/STI/HCV prevention, counseling, or community health
  • Strong understanding of LGBTQ+ health, TGNCNB communities, and harm reduction practices
  • Experience with outreach, case management, and community engagement
  • Strong communication and facilitation skills
  • Ability to work evenings and weekends as needed
  • Valid driver’s license and reliable transportation

Preferred

  • HIV testing certification (or willingness to obtain within 90 days)
  • Experience supporting or supervising staff
  • Spanish language proficiency
