๏ปฟKey Responsibilities

  • The Platform Coordinator will need to have organizational skills with the ability to effectively prioritize and complete tasks to support Care Coordinators with platform referrals.
  • Coordinates to process patient referrals from Navi, Allscripts, Epic NCH, and Power chart, as well as other referral platforms.
  • Must be detail-oriented, possess the ability to multitask, and be open to cross-functional training in all departments.
  • Displays a high level of patient focus that works directly with Care Coordinators, Case Managers, Physicians/PCP(s), insurance representatives, outside vendors, and other medical personnel.
  • Pulls referrals off platforms to accept/decline, Reviews documents, confirms, and processes the referral through Intake using Pinnacleโ€™s EMR.
  • Responds to Care Coordinator inquiries regarding referral status promptly.
  • Responds to all inquiries with a resolution to concerns and or discrepancies.
  • Coordinates and communicates with the appropriate team regarding any changes or updates to the patientโ€™s payer source on time.
  • Meet productivity goals as assigned by the direct supervisor.
  • Complies with HIPAA regulations on all accounts.
  • All other responsibilities assigned by leadership.
