Position Summary
Provides direct support to families in the Family Preservation Program to promote child safety, stability, and well-being. Works collaboratively with case managers, therapists, and community partners to ensure families receive necessary services and meet case plan goals.
Key Responsibilities
  • Provide daily direct and indirect support to families in home, school, and community settings
  • Coordinate community resources (e.g., healthcare, childcare, housing, employment, benefits)
  • Collaborate with case managers and therapists to implement case plans
  • Offer emotional support, coaching, crisis intervention, and parenting guidance
  • Assist families with accessing services such as SNAP, Medicaid, SSI, and employment support
  • Maintain accurate and timely documentation and prepare records for audits
  • Identify and address barriers to family engagement and service success
  • Transport clients as required and attend court proceedings when applicable
  • Support ongoing communication between families and service providers
  • Promote teamwork, cultural sensitivity, and agency values
Qualifications
  • High School diploma or GED required
  • Valid driver's license and ability to transport clients
