This position is Full Time working Monday-Friday 8am-5pm.

What you will do:
The Hospice Social Worker is a professional who coordinates and directs support services to hospice patients in their home environment (private residence, skilled nursing facility, assisted living facility, hospital, etc.) based on individual needs. The Hospice Social Worker facilitates the provision of supportive services to assist to meet the needs of the patient and caregivers, with a holistic approach, keeping with the patient and caregiver’s beliefs and goals of care. Provides consultation and education for patients, caregivers, and the Hospice IDG Team. The Hospice Social Worker coordinates with the Hospice IDG Team, patient, caregivers, providers, Medical Director, referral sources, community sources and the Hospice IDG members to work toward plan of care goals. The Hospice Social Worker documents in the patient’s medical records in a timely manner according to agency requirements. The Hospice Social Worker provides patient care based on provider’s orders, departmental policies and procedures, regulatory guidelines and best practice. The Hospice Social Worker attends and actively participates in IDG and team meetings. The Hospice Social Worker participates in Holiday staffing.
Required Education: 
Master’s degree in Social Work

Required Skills:
Generally accepted standards of practice in respective discipline. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.) Ability to be open, sensitive, flexible and ecumenical. Must have demonstrated experience in working with patients and caregivers dealing with life-threatening illness and end of life. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.

Preferred Skills:
Experience with Home Care Home Base; Experience with smart phones and smart devices
 
Required Licensure/Registration/Certification:
Current Texas state license in appropriate discipline; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation.

Preferred Licensure/Registration/Certification:
Hospice and Palliative Care certified; Advanced education or certification in area of specialty

Required Experience:
Two years’ experience in a healthcare setting in respective discipline

Preferred Experience:
Experience in respective discipline in a hospital or other healthcare setting; Experience in Case Management; Supervisory position in healthcare; Hospice or Home Care experience
