Job description

As a Host Home Caregiver for Golden Heart you will share home and life with individual(s) with Developmental Disabilities (IDD) The agreement is for one(1) full year, the host home caregiver may sign another agreement at the end of their signed agreement.
If the host home caregiver decides to leave, they must give a 3 month notice in writing. As a host home caregiver, you may own or rent your home and provide them hands-on support, and ongoing daily care. You can help someone who has some daily challenges and make a difference in their life as they become part of your family. The caregiver will provide a furnished basement or bedroom to the member and they will have access to the kitchen and living areas.
If you are fluent in any languages please make sure you mention all the languages you know.
Requirement(s)
Must be willing to have staff come by unannounced, and announced home visits to check to see how clients are doing.
Must be willing to have video camera(s) in the home. Common areas such as the living room and kitchen.
Must be willing to implement and monitor the person-centered plans for the people we serve.
The independent contractor must comply with the documentation policy before getting paid for the services that have been provided to the member(s).
Must be willing to sign an agreement as a contractor and follow agreed-upon statutes laid out in the agreement.
Qualifications for the position are as follows:
  • Must have experience with providing personal care
  • Must have experience working with individuals with intellectual and developmental disabilities
  • Must have the ability to use adaptive equipment for transfers, Hoyer lifts, and bathing, training provided
  • Should have a High school or equivalent education (Required)
  • Must have a valid Driver's License and Vehicle Insurance
  • Must have their own vehicle
  • Must complete training for Medication Administration, CPR/First Aid and maintain ongoing training as deemed necessary by agency standards
  • No pets or full-time children (preferred by some clients, but we will match Host Home Providers with clients who fit your circumstances)
  • Good sense of humor, patience, and a dedicated personality are a plus
  • Companions must be willing to be onboarded by Golden Heart and pass the basic requirements
  • The above includes completing a full application and passing background and reference checks

Duties (include but are not limited to):
Assist with daily living skills and social skills
Assist with independent living skills.
Assist with transportation needs.
Enjoy community outings together.
Support and help preparing and serving meals.
Support and help with medications.
Support and help with hygiene and grooming as needed.
Support and help with bathing and toileting as needed.
Maintain complete, and accurate documentation for the member each day.
Provide general supervision, guidance, and security for the member.
Allow the client the ability to succeed in their life goals.
Offer client the ability to function as an independent adult in their home setting, work setting, community settings.
Offer safety, security, respect, independence, and autonomy to our adult clients, with developmental and or intellectual disabilities.

Compensation: $45,000.00 - $60,000.00 per year
