Hackensack Meridian Health provides expert Hospital From Home care across New Jersey, delivering advanced treatment for patients who qualify for inpatient-level care without the need for a hospital stay. Patients in communities served by Hackensack University Medical Center, Jersey Shore University Medical Center, JFK University Medical Center and Ocean University Medical Center can access nationally recognized specialists close to home. The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.  The Hospital at Home Per Diem Nurse Practitioner will serve in a dual capacity. The APN will take a lead role in identification and assessment of potential Hospital at Home referrals, in collaboration with the Virtual Hospitalist. The APN must be committed to providing high-quality, evidence based, patient centered care to a diverse patient population in a non-traditional setting. The Hospital at Home Advanced Practice Nurse Provider (APN) will serve in a dual capacity. The APN will take a lead role in identification and assessment of potential Hospital at Home referrals, in collaboration with the Virtual Hospitalist.

Schedule will be at minimum 4 shifts per month (three shifts -weekend days)

Shift hours are typically 8am-5pm.
