Medford, Oregon, Nurse Practitioner/Physician Assistant Primary Care Job

- Seeking a board certified Nurse Practitioner or Physician Assistant
- Permanent, W-2 employed position
- Full-time (5 days/week) or part-time (3 days/week) opportunities available
- Monday–Friday, 8:00 AM–5:00 PM
- Outpatient clinic position with occasional in-home visits for Medicare patients when needed
- Entire care team is employed by the organization with standardized policies and procedures
- Perform annual wellness visits for senior Medicare patients
- Conduct HCC coding, risk adjustment, medication reconciliation, screenings, and discharge planning
- Collaborate with social workers, care managers, physician advisors, and interdisciplinary care teams
- Average daily volume:
- 8–10 clinic patients
- 5–6 in-home visits when scheduled
- Complete EMR documentation within 72 hours
- Home visits are generally within a 30-mile radius when required

Requirements:
- Board Certified Nurse Practitioner or Physician Assistant
- Oregon license (or eligible)
- Minimum two years of experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or Palliative Care
- DEA and BLS/CPR certification
- Experience with Medicare HCC coding or managed care preferred
- Valid Oregon driver's license
Compensation & Benefits:
- Competitive salary: $125,000–$150,000 annually (prorated for part-time)
- Full-time: $10,000 sign-on bonus
- Part-time: $6,000 sign-on bonus
