Medford, Oregon, Nurse Practitioner/Physician Assistant Primary Care Job

Clinical Role:

  • Seeking a board certified Nurse Practitioner or Physician Assistant
  • Permanent, W-2 employed position
  • Full-time (5 days/week) or part-time (3 days/week) opportunities available
  • Monday–Friday, 8:00 AM–5:00 PM
  • Outpatient clinic position with occasional in-home visits for Medicare patients when needed
  • Entire care team is employed by the organization with standardized policies and procedures
  • Perform annual wellness visits for senior Medicare patients
  • Conduct HCC coding, risk adjustment, medication reconciliation, screenings, and discharge planning
  • Collaborate with social workers, care managers, physician advisors, and interdisciplinary care teams
  • Average daily volume:
    • 8–10 clinic patients
    • 5–6 in-home visits when scheduled
  • Complete EMR documentation within 72 hours
  • Home visits are generally within a 30-mile radius when required
  • Opportunity to make a meaningful impact caring for Medicare and high-risk senior populations

Requirements:

  • Board Certified Nurse Practitioner or Physician Assistant
  • Oregon license (or eligible)
  • Minimum two years of experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or Palliative Care
  • DEA and BLS/CPR certification
  • Experience with Medicare HCC coding or managed care preferred
  • Valid Oregon driver's license
