The In-House Therapy Team at Shore Gardens Rehabilitation and Nursing Center, a premier long term skilled nursing facility located in Ocean County, New Jersey is actively looking for a caring, dedicated Physical Therapist to provide quality care to its residents. The Physical Therapist evaluates and treats patients/residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient’s medical records. Physical therapy services include: treatment planning & implementation, discharge planning, testing patients’ physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.

 PT Duties Include:

·         Evaluate Residents conditions & develop appropriate physical rehabilitation plans.

·         Help Residents improve their mobility.

·         Relieve Resident’s pain using modalities, hands-on therapy, therapeutic exercises & stretching.

·         Teach Residents how to use physical therapy equipment.

·         Help Residents through their tasks and monitor their performance.

·         Maintain Resident confidence & protects the facility by keeping information confidential.

·         Complies with federal, state, and local legal and professional requirements

·         Contributes to team effort.

·         Submit all billing logs and necessary information on a timely basis.

·         Supervising up to 2 PTA's and cosigning all notes


PT Requirements Include:

·         Graduate of an accredited Physical Therapy Program

·         Current New Jersey PT License

·         1 Year working experience as a Physical Therapist preferred, but new grads considered.

·         SNF Experience preferred, but not required

·         Hands-on experience with Net Health (Rehab Optima) preferred

·         Current knowledge of treatment practices per discipline and drive to continually learn and grow in                          the profession

·         Professional behavior

·         Ability to manage patients with different types of personalities

·         Excellent interpersonal communication skills
