Hybrid Resource Counselor/Housing Focused Case Manager

We are currently seeking a Hybrid Case Manager to provide services that help individuals experiencing homelessness, secure and sustain housing. This position plays an essential role in delivering short- and long-term case management, supportive services, and access to job training and placement opportunities.

What You'll Do

  • Manage a caseload of up to 6 individuals with intensive and outcome driven supportive services while delivering these services utilizing a Housing First approach.
  • Incorporate CalAIM care coordination and housing stabilization strategies into individualized housing and care plans, in collaboration with healthcare, behavioral health, and community-based partners. 
  • Provide housing-focused case management in one-on-one and group settings. 
  • Identify and address barriers to housing stabilization, including chronic homelessness, mental and physical health needs, substance use disorders, under-employment, lack of income, and other systemic barriers. 
  • Skillfully prevent and/or resolve conflicts skillfully using a strength-based and trauma-informed approach. 
  • Respond to urgent and crisis situations, including coordination with EMTs, fire, law enforcement, psychiatric services, and community partners as needed. 
  • Provide supportive services, guidance, and referrals to community resources. 
  • Engage program participants regularly and consistently to support program participation and progress toward housing stability while remaining flexible and creative around interventions.
  • Support in managing the operation of our weekly referral log and intake calls; ensuring referrals are accurately documented, updated, and responded in a timely manner.

What We're Looking For

Required

  • Bachelor’s degree in social work, psychology, social welfare, or a related field or two (2) years of experience in a related residential or housing program.
