Job description

Overview

Catholic Health Home Care is looking for passionate, dedicated Speech Language Pathologists to provide comprehensive care to patients in Suffolk County including Bohemia, Brentwood, Fire Island, Holbrook, Islip, Lake Ronkonkoma, Ronkonkoma, Sayville, & surrounding towns.  

Responsibilities

Our Home Health Speech Language Pathologists will:

  • Assess, plan, develop and provides the patient or responsible caregiver a home health plan of care for speech language pathology.
  • Identifies patient needs for other disciplines and services. 
  • Coordinates and evaluates the plan of care with all other disciplines and physicians for an assigned caseload. 
  • Instructs the patient or responsible caregiver on the projected care plan and expected outcomes as directed by the physician's treatment plan.
  • Detailed documentation of visits in EMR system.

Minimum Qualifications:

  • Licensed and current registration to practice as a Speech Language Pathologist in New York State required.
  • Minimum of one year post-licensure experience in rehabilitation with adult speech/language impairment populations.
  • Good computer skills - Electronic Medical Record experience required.
Pay RangeUSD $75.00 - USD $85.00 /Hr.Qualifications

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
