What are we looking for?

On behalf of our home health agency client, one of the largest agencies in the Phoenix Area, we are looking for multiple Registered Nurses to care for and focus on patients in a small radius around the candidate's residence. As a home health nurse, you'll work one-on-one with your patients in the comfort of their homes. This allows you to develop deeper relationships and work in a more personal setting than a medical office or hospital.

Responsibilities

  • Self-manage, schedule and perform visits based on assigned patient base
  • Completing an initial home health assessment of patient and family to determine home care needs.
  • Providing a complete physical assessment and history of current and previous illness.
  • Conducting home visits as assigned, assessing the patient's condition, and progress toward goals, and providing patient care, teaching and other skilled interventions as ordered.
  • Regularly re-evaluating patient nursing needs, as part of nursing visits.
  • Initiating appropriate preventive and rehabilitative nursing procedures.
  • Administering medications and treatments as prescribed by the physician.
  • Providing health care instructions to the patient as appropriate per assessment and plan of care.
  • Communicating needs to the team as part of the care plan development; assists in implementing before discharge of the patient.
  • Informs personnel of changes in the condition and needs of the patient.
  • Counsels the patient and family/significant others in meeting nursing and related needs.
  • Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients promptly as per Agency policy.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition by Agency defined time frames.
  • Assuring proper maintenance of clinical records in compliance with local, state and federal laws.

Skills

  • Graduate of an accredited school with at least one (1) year of nursing experience
  • Preference will be given to candidates with experience working in home health, hospice & skilled nursing, but not required)
  • Ability to autonomously with little supervision
  • Passionate about patient care and making an impact by treating patients in their own homes
  • Must have an Arizona license in good standing and a Driver's License.
  • Reliable Transportation, current CPR or BLS Card

Location

We are looking for candidates across the entire Phoenix Area. Candidates will be grouped in 3 regions, from West to East and will be travelling to see patients within a 25-mile radius of their place of residence.
