Job description

Overview: Consistently provides one to one assistance and instruction for the clients to ensure basic health, safety, and community integration.

Duties:
• Implementing client Individual Support Plan (ISP) and Treatment Program.
• Assisting in all daily paperwork including: reports, dailies, seizure logs, current medication sheets, and SIR's information, etc.
• Accurate and proficient charting and documentation daily.
• Assist in teaching the clients about personal care, household chores, daily living, community resources, safety awareness, social and recreation opportunities in the community.
• Assist clients to address their medical, dental and mental health needs by scheduling appointments, coordination of transportation and utilizing standard medication procedures.
• Assist the clients in addressing issues and challenges by role-modeling appropriated responses.
• Assist clients with budgeting, grocery shopping and meal preparation.
• Assist in household maintenance including but not limited to yards and basic landscaping needs.
• Attend any relevant meeting as assigned by Administrator and/or the Executive Directors.
• Other duties as assigned by the Executive Directors.
Ancillary Functions:
• Represent Tailored Living Choices in a professional manner at all times.
• Establish and maintain a constructive and professional relationship with clients and their circle of support.
• Work independently with the flexibility to work with any clients as directed by a manager.
• Follow written and verbal instructions in English from Executive Directors and Administrator.
• Remain calm in situations of high stress.
• Promote and maintain a congenial supportive environment for the clients.

• Some lifting involved.


• 18 years of age or older with a minimum of a high school diploma or the equivalent.
• Minimum of one year’s experience working specifically with individuals with developmental disabilities is highly desirable.
• Ability to read, write and comprehend English.
• Follow written and verbal instructions in English from supervisors.
• Must have independent and reliable transportation, valid auto insurance and clean DMV.
