Job description
This is a single application process used across multiple mental health and substance use services. Instead of advertising each role separately, we maintain a talent pool so we can hire quickly when needs arise.

- Intensive, multidisciplinary community treatment for individuals with severe and persistent mental health needs
- High-frequency outreach, crisis response, and long-term stabilization support
CST (Community Support Team)
- Community-based mental health support focused on recovery, independence, and ongoing engagement
- Structured support for individuals stepping down from higher intensity services or needing preventative intervention
IIH (Intensive In-Home Services)
- Family-focused, in-home therapeutic support for children, adolescents, and caregivers
- Support aimed at reducing risk of out-of-home placement and improving family functioning
Personal Care Services
- Daily living support (ADLs) for individuals with mental health, disability, or substance use recovery needs
- Assistance with routines, safety, and maintaining independence
Tailored Care Management
- Care coordination for individuals with complex behavioral health and social needs

Education, experience & licensing

- Professional roles: bachelor's degree in social work, psychology, counseling, or related field

- Direct experience in mental health or substance use services
- Trauma-informed, person-centered approaches
Certifications may include CPR/First Aid, crisis intervention training, or other state-specific behavioral health requirements depending on role.
