JOB SUMMARY

Under the intermittent supervision of the Ambulatory Nursing Services Manager, the Ambulatory Nursing RN provides direct and indirect patient care in the ambulatory/primary care setting. Assesses patients, using nursing judgement and appropriate screening protocols to determine unmet healthcare needs. Applies nursing diagnoses to define unmet needs. Organizes interventions to address identified needs, including facilitating referrals, obtaining medical orders, or providing education and case management support for the patient. Effectively communicates identified needs and care plan with the multidisciplinary care team including providers, pharmacists, dieticians, social workers, support groups, and more. Evaluates the effectiveness of interventions and adjusts the care plan accordingly. Chemotherapy experience a plus as services will be expanding to include chemotherapy administration in the future.

ESSENTIAL FUNCTIONS

Perform patient assessment considering needs specific to the standard of care for patient’s age. Uses appropriate screening tools effectively.

Demonstrates understanding of pathophysiology, disease processes, and treatment standards. Applies nursing diagnoses to identified needs.

Formulates a care plan based upon identified needs and facilitates interventions and referrals as appropriate.

Performs interventions in a competent manner, applying evidence-based standards of care and protocols to guide actions. Knowledgeable regarding medications, indications, interactions, and contraindications, considering age of the patient and their clinical condition. Evaluates effectiveness of all interventions.

Administers vaccines and medications in clinic per protocols or with authorized outpatient medication orders.

Formulates individualized teaching plans, evaluating effectiveness of learning. Patient and family interactions are professional.

Familiar with Policies, Procedures, and Protocols. Care and Documentation meets current standards of practice and complies with all such guidelines.

Other duties as assigned to support the overall effectiveness of the department and organizational performance in accordance with Curry Health Network's Mission, Vision, and Shared Values.

MINIMUM JOB REQUIREMENTS

Education & Experience

  • Current Unencumbered Oregon State Registered Nurse Licensure. BSN Preferred.
  • Current AHA BLS and ACLS certification.
  • Minimum 2 years’ experience in Ambulatory or Acute care setting
  • Knowledgeable about wellness promotion
  • Wound care certificate preferred

Chemotherapy certification preferred

Required Knowledge, Skills & Abilities

  • Computer skills are required, specifically using MS Office (Word, Excel, Outlook); prior use of an Electronic Medical Record (EMR) for documenting in patient records is preferred.
  • Knowledge and ability to deliver acute and chronic wound care.
  • Excellent communication skills, clinical judgment, critical thinking, and organizational skills are required.
  • A demonstrated ability to multi-task is required.

Whole-person perspective to case management is critical.
