Pay: $40-$50/per Nursing visit / $45/hr for Care Management

Location: Merrillville, IN
Service Area: Lake and Porter Counties
Job Type: Part-Time/PRN

About the Opportunity

Firefly Home Health and First Call Care Management are seeking a compassionate, dependable, and experienced Registered Nurse (RN) to support clients throughout Lake and Porter Counties.

This is a unique dual-employment opportunity for an RN interested in providing both in-home healthcare services and geriatric care management.

The selected RN will have the opportunity to work with:

  • Firefly Home Health, conducting in-home healthcare visits.

  • First Call Care Management, coordinating and managing care for older adults and their families.

This position will begin on a part-time or PRN basis, with the potential for additional hours as the client base grows.

This position is specifically for a Registered Nurse. Licensed Practical Nurse applicants will not be considered.

ResponsibilitiesFirefly Home Health
  • Conduct in-home healthcare visits throughout Lake and Porter Counties.

  • Complete nursing assessments and evaluate clients’ healthcare needs.

  • Monitor and document changes in clients’ conditions.

  • Provide nursing services in accordance with individualized plans of care.

  • Communicate with clients, families, physicians, caregivers, and other members of the care team.

  • Educate clients and families regarding care needs, treatments, medications, and safety.

  • Complete accurate and timely clinical documentation.

  • Follow all applicable home-health regulations, policies, and professional nursing standards.

First Call Care Management
  • Conduct comprehensive assessments of clients’ health, functional abilities, psychosocial needs, and living environments.

  • Develop, implement, and monitor individualized care plans.

  • Coordinate healthcare services, home-care services, and community resources.

  • Serve as a liaison between clients, families, healthcare providers, and other members of the care team.

  • Advocate for clients’ needs, preferences, safety, independence, and overall well-being.

  • Provide support and guidance to families navigating healthcare systems and care decisions.

  • Monitor clients’ conditions and adjust care plans as their needs change.

  • Maintain accurate and up-to-date client records.

  • Ensure compliance with applicable laws, regulations, ethical standards, and company policies.

Qualifications
  • Active and unrestricted Indiana Registered Nurse license.

  • Registered Nurse applicants only; LPN applicants will not be considered.

  • Minimum of two years of experience in home health, geriatric care, case management, or a related healthcare setting.

  • Geriatric Care Manager or Aging Life Care Manager certification preferred.

  • Strong knowledge of healthcare systems and resources available to older adults.

  • Excellent assessment, communication, documentation, and care-planning skills.

  • Strong organizational skills and the ability to manage multiple clients and responsibilities.

  • Ability to work independently while collaborating with families and healthcare professionals.

  • Valid driver’s license and reliable transportation.

  • Ability to travel throughout Lake and Porter Counties.

Schedule

This position will begin on a flexible part-time or PRN basis. Hours may fluctuate based on home-health visits, care-management assignments, and client needs.
