Description

American Traveler is seeking an experienced RN Care Manager for an acute care hospital setting requiring 2+ years of RN experience and NC or compact RN licensure.

Details
  • Work in a hospital-based Care Management department supporting a 20-25 bed unit
  • Patient population includes Cardiology, Nephrology, Trauma Surgery, Ortho Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown services
  • Days shift schedule from 8:00am to 5:00pm
  • 36 hours per week across 3 shifts at 12 hours per day
  • No on-call responsibilities
  • No weekend requirements
  • No holiday coverage required
  • Uses Epic EMR
Requirements
  • Active RN license in North Carolina (Standard or Compact)
  • Minimum of 2 years of healthcare experience as a Registered Nurse
  • Nursing Diploma, ADN, or ASN from an accredited nursing school required; BSN or MSN preferred
  • Strong assessment and critical thinking skills required
  • Prior experience as a case manager, care manager, or discharge planner in an acute care hospital setting preferred
  • Epic EMR experience preferred
  • Experience in academic or teaching hospital settings is helpful
Additional Information
  • Conduct comprehensive patient assessments and document plans of care in Epic per departmental guidelines
  • Participate in daily Care Management Touchpoints and Communication and Patient Planning (CAPP) meetings for assigned units
  • Attend and present at weekly Complex Care Meetings to collaborate on complex patient cases and identify trends
  • Coordinate with Social Workers, Utilization Managers, and Care Management Assistants to support patient progression and discharge planning
  • Identify and address barriers to discharge, coordinate family meetings, and provide education on community resources
  • Monitor observation patients and assess discharge readiness on an ongoing basis
  • Identify required authorizations for post-discharge services and coordinate referrals to post-acute providers
  • Collaborate closely with an interdisciplinary care team to enhance patient management and continuity of care
