The Coordinated Caregiving (CCG) Registered Nurse plays a critical role in ensuring the safe and effective administration, monitoring, and documentation of medications for individuals served in GT Independence's North Carolina's Coordinated Caregiving Program. In addition, the RN is responsible for the oversight, delegation, and training of specialized skilled cares, ensuring that all clinical supports are delivered in accordance with best practices, regulatory standards, and the unique needs of each individual served.
This position collaborates with Case Management Entity RNs, Case Managers (CMs), and community partners to support clinical needs and medication safety in accordance with GT Independence policies, DHS regulations, and Coordinated Caregiving guidelines.
Key Responsibilities
Clinical Coordination & Assessment
  • Collaborate with the Case Manager to obtain and verify the most up-to-date medication list from the client's Primary Care Provider.

  • Conduct in home assessments to evaluate clients' clinical needs and ensure alignment with GT Independence policies.

Medication Management
  • Complete and maintain accurate Medication Administration Records (MARs) based on the verified medication list.

  • Monitor CCG team documentation on MARs for accuracy and compliance.

  • Provide medication administration and monitoring education and support to CCG staff, ensuring adherence to Medicaid guidelines, DHS regulations, and GT Independence policies.

  • Ensure proper procedures are followed for medication storage, handling, and disposal

Training & Oversight
  • Deliver competency-based training to CCG staff on medication administration and medication monitoring, including refresher training annually.

  • Educate CCG staff on the "Six Rights" of medication administration: right person, right drug, right dose, right route, right time, right documentation.

  • Review completed MARs monthly and collaborate with CCG participant to determine if employee counsel is warranted based on documentation accuracy and compliance.

  • Support and oversee CCG staff in the preparation and documentation of medications for Leave of Absence, ensuring procedures align with GT Independence policies and are accurately recorded.

  • Provide oversight, delegation, and training related to specialized skilled cares, ensuring CCG staff are competent and confident in delivering individualized clinical supports in accordance with regulatory guidelines and person-centered care plans.

Documentation & Compliance
  • Ensure all medications (prescription, OTC, alternative, supplements, etc.) are documented in the MAR.

  • Maintain annually updated records including signed consents, disclosures, and detailed medication profiles.

Compliance & Quality Assurance
  • Collaborate with the Director of Operations and GT Independence's Compliance Team to ensure all medication administration, monitoring, and related care practices align with CARF standards and internal policies.

  • Participate in regular reviews and audits of documentation and procedures to uphold regulatory compliance and promote continuous quality improvement.

  • Support staff in understanding and implementing best practices for medication safety, documentation accuracy, and regulatory compliance.

  • Assist in identifying and resolving compliance concerns with the Director of Operations and contribute to the development of corrective action plans when necessary.

Community Collaboration & Education
  • Collaborate with community partners to identify and address educational needs for CCG staff.

  • Support persons served, parents, and guardians in understanding medication benefits, side effects, and informed decision-making.

Education
  • Current and unencumbered RN license in the state of North Carolina

Experience and Qualifications
  • Experience delivering compassionate, individualized care, focused on supporting people through a wide range of health journeys and personal goals.

  • Strong understanding of medication administration protocols and regulatory compliance.

  • Excellent communication and organizational skills.

  • Ability to work independently and collaboratively in a remote and field-based environment.

  • Must pass background checks and meet training requirements prior to medication-related tasks.

  • Must be able to travel to client homes and community locations as needed.

  • Familiarity with electronic health records and telehealth platforms are preferred.

Work Environment
  • Work is performed in an office environment as well as community-based settings, primarily in participants' homes. May include light to moderate lifting, bending, and walking. Traveling is required.
