Clover Home Health is hiring a Full-Time Registered Nurse (RN-Home Health) to support clients on the north-side of Indianapolis.

Our ideal Full-time Home Health Registered Nurse candidate must have home health nursing experience. This Registered Nurse candidate needs to be proficient with OASIS charting and use of EMR systems (AXXESS preferred). This is a Full-Time Salaried position.

Requirements:

  • Current nursing license required for the state of Indiana

  • Degree in Nursing from an accredited nursing school, university or college

  • Valid driver's license and automobile insurance

  • Current in-person CPR certification

  • TB test screening (past 30 days)

  • Two years Home Health nursing experience (required)

  • Two years OASIS experience (required)

  • Tech savvy, able to learn and navigate several platforms. Proficient with Apple products.

  • Perform routine duties independent of supervision but consults with others as required

  • Excellent communication and listening skills

  • Use of AXXESS, EMR system (preferred)

  • Excellent time management and communication skills (required)

Responsibilities:

  • Complete new patient intakes

  • Direct patient care (SOC's, ROC; Re-Certs, Discharge, Transfers, Nurse Visits, and ALL OASIS documentation)

  • Document complex medical information to outline and improve health results that help determine immediate and routine care for the client

  • See patients and clients, as needed

  • Work collaboratively with the DON/ADON

  • Communicate patient conditions and collaborate with the DON/ADON to deliver care when patient needs evolve

  • Work to decrease re-admissions by promoting preventative care and ensuring continuity of care

  • Additional duties as assigned

Benefits:

  • Salary $75,000-$85,000

  • PTO - 10 days after 90 days of employment

  • 401k - eligible after one year of employment

  • $1000 sign-on bonus after 180 days of employment
