Clinical Documentation Coordinator opening at Anew Care

Position is onsite in Indianapolis, IN!

Anew Care is seeking a highly organized, detail-oriented professional to support our Home Health, Hospice, and Palliative Care teams in documentation management, compliance tracking, and operational coordination. This role is essential in ensuring timely document completion, regulatory compliance, and operational efficiency across our clinical services.

Key Responsibilities:

  • Documentation Management

    • Send clinical and administrative documents via EMR e-fax, DocuSign, and healthcare portals for required signatures
    • Track outstanding documents and proactively follow up with physicians, facilities, and internal team members
    • Maintain organized tracking systems to ensure timely completion
    • Escalate delays that may impact billing or compliance
  • Medical Records & Audit Support

    • Process medical records requests
    • Assist with documentation gathering for Targeted Probe and Educate (TPE) reviews and other audits
    • Ensure records are complete and submitted within required timelines
  • IT Equipment Tracking

    • Maintain accurate tracking logs of employee-issued equipment (laptops, tablets, phones, etc.)
    • Record serial numbers, assignment dates, and retrieval upon separation
    • Coordinate with leadership to ensure accountability
  • Additional Administrative Support

    • Assist leadership with documentation workflow improvements
    • Identify opportunities to streamline follow-up and tracking processes

Qualifications:
