Job description

JOB DESCRIPTION:

Community Inclusion Specialist
PURPOSE:

To develop meaningful relationships with the people we serve, their families and the communities in which they live. This means you are called to be creative and compassionate in all that you do; providing direct care, delivering guidance and teaching and empowering the people we serve to be their most independent selves.


JOB RESPONSIBILITIES:

  1. Ensure the health and safety of the clients we serve, at all times.
  2. Assist clients in activities and functions of daily living including but not limited to restroom/hygiene assistance, mealtime prep and eating, when applicable medication administration, maintaining healthy and safe social boundaries.
  3. Engage in meaningful activities as directed throughout the day with clients to ensure clients are having fun and building skills. Assist with completing individual service plan goals for assigned participants, completing goals and other required documentation daily.
  4. Participate in scheduled staff meetings, training opportunities, and maintain required competency-based certifications (First Aid & CPR)
  5. Transport the people we serve to and from the Empowered Center and to various locations throughout the community.
  6. Perform building and vehicle cleaning duties to maintain a safe and sanitary work environment for the people we serve and fellow staff.
  7. Perform any other duties related to Empowered Services as may be assigned by the Branch Manager.

QUALIFICATION REQUIREMENTS:

  1. Must be 18 years of age
  2. High school diploma or GED (preferred)
  3. Criminal Background Check
  4. Drug Screen and Physical
  5. Certification in CPR and First Aid.
  6. Driverโ€™s License and Bureau of Motor Vehicle Driver History Review
  7. A commitment to providing excellent care and customer service.
  8. Ability to adapt to changing circumstances, critical thinking skills and creativity.
  9. Employee must be physically capable of bending, reaching, transferring, assisting with transfers and lifting people who require physical assistance. Employee must have the ability to lift 35lbs.
