Position: Chronic Care Management Specialist

Location: Raleigh, NC (on-site position)

Schedule: Care Navigators can work Sundays - Saturdays from 8am to 9pm. We are looking for at least 36 hours per week of availability (Full Time and eligible for benefits).

What You'll Do:

- Be a health hero to over 500 patients across North Carolina

- Build lasting relationships through regular check-ins and support calls

- Use state-of-the-art remote monitoring tools to track patients' health in real-time

- Collaborate with our clinical pharmacists to optimize patient care plans

- Educate and motivate patients to achieve their health goals

- Be the friendly voice and listening ear that patients look forward to hearing from
