What You'll Do
  • Manage a home health caseload throughout St. Paul, with on-call and weekend visits within approximately 35 miles of your home.
  • Complete OASIS assessments and individualized plans of care.
  • Deliver skilled nursing care and patient education.
  • Collaborate with physicians and interdisciplinary team members.
  • Help patients safely recover and remain independent at home.
  • Maintain accurate, timely clinical documentation.

What We're Looking For
  • Active Minnesota Registered Nurse (RN) license.
  • 3+ years of RN experience (home health experience preferred).
  • Strong clinical judgment and critical thinking skills.
  • Excellent communication and organizational abilities.
  • Reliable transportation for local travel.
