Job description

SUMMARY
As a Physician Assistant in Geriatrics, you'll provide comprehensive primary and acute care to older adults across both Long-Term Care and Assisted Living settings. This role offers the opportunity to build meaningful relationships with patients while managing a diverse range of acute and chronic conditions. You'll work alongside a supportive team with strong onboarding and collaboration with nursing, respiratory therapy, facility staff, and experienced geriatric physicians.
STATUS: Full Time
LOCATION: Multiple Locations (Monroe County)
DEPARTMENT: Geriatrics
SCHEDULE: Days, Monday-Friday

ATTRIBUTES

RESPONSIBILITIES
  • Patient Care. Provide primary and acute care for geriatric patients, including assessment and management of acute and chronic conditions, physical examinations, diagnosis, treatment planning, medication management, and follow-up care
  • Long-Term Care. Provide care 2-3 days per week at Unity Living Center, including management of long-term care residents and patients requiring long-term ventilator support in collaboration with respiratory therapy
  • Assisted Living. Provide primary care 2-3 days per week to residents in assisted living settings, working closely with facility staff and the Geriatric Medical Director
  • Care Coordination. Collaborate with nursing, respiratory therapy, facility staff, physicians, and other members of the care team to coordinate comprehensive patient care
  • Documentation. Maintain accurate medical records, complete required documentation, and monitor patient response to treatment

EDUCATION:
BS: Physician Assistant (Required)
LICENSES / CERTIFICATIONS:
PA-C - Certified Physician Assistant - National Committee on Certification of Physician Assistants (NCCPA), RPA - Registered Physician Assistant - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$100,000.00 - $150,000.00
