Job description

  • GENERAL SUMMARY
    • The Hospital Violence Intervention Program (HVIP) Coordinator provides program coordination for a multi-disciplinary, hospital-based violence intervention program, dedicated to reducing injury, re-injury and mortality rates among youth and adults by providing victims of gun violence or other types of intentional violence with support and resources during their hospital stay and beyond. The Coordinator is responsible for program development to include establishing goals for the program and project planning. Reports to the Medical Social Work Manager.
  • ESSENTIAL DUTIES AND RESPONSIBILITIES
    • Administrative Responsibilities:
    • Provides hospital and community-based services to patients and families within the CHKD Health System who have experienced intentional violent injury.
    • Works with program evaluators to monitor and assess performance in relation to stated program objectives.
    • Manages administrative and fiscal operations for the HPIV program in accordance with our contract.
    • Researches all application processes, compiles application data, determines deadlines, and manages/oversees project research, budget, design and implementation.
    • Responsible for data, grant management, and other reporting as required by the grant-funding source.
    • Identifies, disseminates, and assigns duties to the HPIV Medical Social Worker as well as provide their clinical supervision.
    • Works closely with the Trauma Team and the Trauma Wellness Coordinator to ensure that the patient and family are receiving maximum support, and encourages adherence to treatment recommendations.
    • Serves as the main point of contact to participating hospitals and funder for technical assistance to include coaching calls and one-on-one virtual coaching.
    • Organizes site visits and shadowing opportunities between participating hospitals.
    • Leads the development and implementation of educational materials and curriculum.
    • Performs other duties as needed.
    • Clinical Responsibilities:
    • Functions, as needed, in the role of providing direct care services to patients and families within the CHKD Health System who have experienced intentional violent injury. It is an expectation the Coordinator will carry a patient caseload and provide coverage for the HVIP Medical Social Worker as needed. The Coordinator will complete a thorough assessment to identify needs of the patient and family members impacted by the incident.
    • Provides immediate crisis intervention support and counseling.
    • Completes mental health evaluations and/or ensure that a referral for mental health support and evaluation is completed.
    • Connects patients and families to the appropriate community resources and services to address concerns and needs identified.
    • Assists in administering and tracking the use of crisis funds serving patient victims of violent injury.
    • Accompanies victims of violent crime within the hospital, criminal justice system, and other systems as needed to support during appointments and meetings.
    • Provides on-going case management and follow-up services to the patient and family for up to one year following the incident.
    • Plans and facilitates community activities as needed to promote program awareness and assist in reducing community violence.
    • Performs other duties as needed.
  • LICENSES AND/OR CERTIFICATIONS
    • Licensed Clinical Social Worker or Licensed Professional Counselor preferred; must be license eligible within six months.
  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
    • Master's Degree in Social Work, Counseling or related licensed eligible field.
    • Minimum of two years' experience in hospital social work, child advocacy, medical social work, or mental health preferred.
    • Previous experience in program development, case management, advocacy, and family systems preferred.
    • Ability to understand and work with at-risk youth and their families.
    • Knowledge of positive youth development pathways and ability to provide assessment, counseling, education/support to at-risk youth and families.
    • Ability to work collaboratively with families and community agencies.
    • Demonstrate proactive problem solving, teamwork, innovative thinking, creativity, patience, and flexibility.
    • Ability to conduct and assist with group activities, such as group sessions, community events, and focus groups.
    • Demonstrate commitment and ability to support and mentor youth in a positive manner
    • Hospital and community experience preferred.
    • Experience working with Child Protective Services, Law Enforcement and court services preferred.
    • Knowledge of community resources, human development, and trauma informed practices.
    • Excellent interpersonal and organizational skills.
    • Must agree to CPS and criminal background check.
    • Dependable transportation required.
    • Increased Security Clearance for job role required: FBI finger printing, criminal background check also includes a search of public sex offender and child abuse websites/registries prior to hire and every five years thereafter. Determination of suitability to work with minors will follow the conditions and guidance specified by Office of Justice (OJP) for grant recipients and subrecipients.
