Care Manager

Department: Client Services – ECM
Reports To: Client Services – ECM Manager
FLSA Status: Non-Exempt (professional role requiring independent judgment)
Pay Range: $23 – $30 per hour
Location: California | Hybrid
Schedule: Full-time, Monday–Friday; occasional extended hours as member needs require

Position Summary

The Care Manager provides Enhanced Care Management services to assigned members. This role focuses on member engagement, care coordination, and documentation in alignment with payer and regulatory requirements.

The Care Manager is responsible for completing required ECM activities accurately, on time, and in accordance with established standards.

Essential Duties and Responsibilities ECM Service Delivery
  • Conduct outreach and engagement with assigned ECM members.

  • Coordinate services and resources to support member needs.

  • Follow established care plans and ECM workflows.

Documentation & Compliance
  • Complete required ECM documentation accurately and on time.

  • Maintain current care plans and assessments.

  • Address documentation gaps promptly.

Risk Identification & Escalation
  • Identify member risks, barriers, or changes in condition.

  • Escalate concerns according to established protocols.

  • Document escalations and follow-up actions.

Collaboration & Communication
  • Partner with Senior Care Managers for guidance and support.

  • Coordinate with operations teams as needed to support continuity of care.

  • Communicate clearly with members and internal teams.

Required Qualifications
  • Minimum 3 years of care management or care coordination experience

  • Strong documentation and organizational skills

  • Ability to manage multiple members and priorities

  • Clear written and verbal communication skills

Preferred Qualifications
  • Experience with California ECM programs

  • Familiarity with EHR and care management platforms

  • Background in healthcare or social services

Skills and Competencies
  • Member engagement and advocacy

  • Documentation accuracy and timeliness

  • Organization and follow-through

  • Systems literacy

  • Professional judgment

Physical and Work Conditions
  • Combination of office, remote, and field-based work
