Job description

Job Title: Enhanced Care Coordinator

Department: Enhanced Care Management (ECM)

Reports To: ECM Director, Assistant ECM Manager

Location: Lindsay, CA

Job Summary: We are looking for a compassionate and organized individual to join our team as an Enhanced Care Coordinator. This is an entry-level, patient-facing position that supports Medi-Cal members enrolled in California's Enhanced Care Management (ECM) program. The Enhanced Care Coordinator help patients connect with health and social services, complete care assessments, and follow up on personalized care plans.

This is a great opportunity for someone who enjoys working with the community, wants to make a difference in patients' lives, and is interested in growing in the healthcare field. Training will be provided.

Key Responsibilities:

  • Conduct outreach calls to patients assigned by our contracted health plans (Anthem and HealthNet).
  • Explain the ECM program and help patients understand the free services available to them.
  • Assist patients in completing health and social needs assessments (over the phone or in-person).
  • Help patients set health-related goals and develop basic care plans.
  • Coordinate services such as transportation, behavioral health referrals, food access, and other community-based resources.
  • Build rapport with patients and their families to support long-term engagement.
  • Maintain accurate case documentation using our care management software.
  • Follow up with patients regularly to ensure progress and update care plans.

Qualifications:

  • Bilingual in Spanish (REQUIRED).
  • High school diploma or GED (preferred).
  • Prior experience working with people in a customer service, healthcare, social work, or community setting is preferred but not required.
  • Strong communication skills (verbal and written).
  • Comfortable working with underserved and diverse populations.
  • Must be organized, punctual, and able to follow workflows.
  • Ability to learn new systems (training provided).

Work Conditions:

  • Full-time, Monday through Friday, 8am to 5pm.
  • Based on-site at our clinic (some off-site outreach may be included in the future).
  • Will require phone-based and in-person interaction with patients.
