Anchor House is hiring a Lead Care Manager to support high-need Medi-Cal members through the CalAIM Enhanced Care Management (ECM) program. This role focuses on overseeing care coordination, supporting care managers, and ensuring quality service delivery.


Responsibilities

  • Lead and support care managers in care coordination efforts
  • Oversee assessments, care planning, and service coordination under CalAIM ECM
  • Collaborate with the ECM Program Manager and external providers
  • Monitor quality measures and reporting requirements
  • Identify program improvements and communicate updates to the team

Qualifications

  • 1+ year of relevant experience required
  • High school diploma or GED required
  • Familiarity with Medi-Cal, CalAIM, or community services preferred
  • Strong communication and organizational skills
  • Proficiency with Google Workspace and basic office tools
  • Flexible availability, including occasional evenings or weekends
  • Multilingual skills a plus
