General Position Description:
The Volunteer and Veterans Coordinator has overall responsibility for the Hospice's Volunteer and We Honor Veterans Program. The Volunteer and Veterans Coordinator is responsible for promoting Infinity Hospice Care's vision and mission to the veteran community, veteran service organizations, to generate inquiries and referrals into our range of services to increase the level of services provided to the veteran community. Volunteers must be used in day-to-day administrative and/or direct patient care roles that meets the needs of the patient and family/caregivers. The Volunteer and Veterans Coordinator is responsible for recruiting, training, retaining, and supervising volunteers along with outreach activities, building strategic relationship and representing the organization to the veteran population as needed to increase awareness of our mission and utilization of our services. The Volunteer and Veterans Coordinator is responsible for creating, maintaining, and reporting volunteer activities to meet regulatory requirements and demonstrate quality of care.
The Volunteer and Veterans Coordinator reports to the Hospice Administrator.
Role Expectations:
- Demonstrates ability to lead and support volunteer program with multiple sites of various sizes to help maintain staffing model and productivity standards.
- Maintains documentation that demonstrates viable and ongoing efforts to recruit and retain volunteers
- Provides training and orientation that is consistent with hospice industry standards and compliance with all local, state and federal laws. Maintains a written description of in-service training provided during the previous 12 months.
- Assesses and maintains documentation of volunteers' skills and competencies.
- Provides guidance related specifically to individual responsibilities.
- Maintains documentation of the cost savings achieved through the use of volunteers including identification of each position occupied by a volunteer; work time spent by volunteers occupying those positions; estimating the dollar costs the Hospice would have incurred if paid employees occupied the position
- Ensures volunteers provide day-to-day administrative and/or direct patient care services in an amount that at a minimum, equals 5% of the total patient care hours of all paid hospice employees and contact staff. Ensures volunteers document to prove the value of work being done for patients and families.
- Participates in the care planning process; documents on the Plan of Care (POC) for all patients receiving volunteer services; reviews and documents on the individualize POC as frequently as the patient's condition requires but no less frequently than every 15 days.
- Performs on-call/after-hours responsibilities and provides volunteer on-call/after-hours services to patients and their families as assigned. In coordination with Inter-Disciplinary Team (IDT) to meet patients' needs 24x7 and as levels of care and intensity of services change including respite, routine, GIP, and continuous.
- Collaboratively develops and maintains policies specific to the volunteer program. Including industry best practices like Death Doula opportunities in communities where aligned.
- Ensures volunteers are aware of and in agreement with their duties and responsibilities and who to report to if they need assistance and instructions regarding performance of their duties and responsibilities.
- Develops the volunteer program in collaboration with the IDG/T and administration.
- Demonstrates knowledge of, and ensures compliance with, all local, state and federal laws relating to the recruitment, training & retention of volunteers.
- Promotes hospice goals, services and philosophy to ensure quality of care
- Establishes a public relations program to foster good working relations with the volunteers and the community
- Carries out other duties as assigned by the IDG/T.
- Provides an environment which promotes respect for patients and their privacy and property.
- Participates in the Hospice's QAPI program.
- Demonstrates commitment, professional growth and competency.
- Follows the Hospice's infection control policy.
- Promotes the Hospice philosophy and administrative policies to ensure quality of care
- Participates in the Hospice's educational in-service programs as required.
- Understanding all aspects of the We Honor Veterans (WHV) program and what is required to maintain the Level 5 status.?
- Participate in all local and/or state level Hospice Veteran Partnership meetings.
- Assist with building and maintaining a positive relationship with the local Department of Veteran Affairs and local veteran organizations as required per the WHV national program.
- Identify veteran hospice patients have a completed Military History Checklist, ensure their plan of care reflects the specified needs of the patient, including recognizing them with either a pinning ceremony or certificate.
- Ensure all WHV program activities are completed and tracked properly for annual submittal to the National Hospice and Palliative Care Organizations/WHV Department to maintain our Level 5 Status.
- Maintain proper inventory of all supplies needed to complete all WHV Ceremonies/Certificate presentations.
- Maintain and track census of all veterans on service.
- Work with the Volunteer Coordinator to maintain the vet-to-vet program.
- Work with Human Resources & Volunteer Coordinator to ensure that all paid and nonpaid employees have taken all veteran related mandatory trainings/in-services required to maintain Level 5 status.
- Assist with managing the We Honor Veterans Department including keeping track of all new veterans that come onto hospice service.
- Timely submission of all required paperwork.
- Other tasks as assigned
Requirements
Education and experience:
- Education: High School Graduate, Bachelor's in Human Services preferred. Licensure: Not required.
- Experience: Experience in recruitment and management of hospice volunteers, preferred.
- Desirable experience in community resources, social work, and volunteerism.
- Skills: Working knowledge of community resources.
- Ability to establish and maintain effective working relationships with the Interdisciplinary Group/Team (IDG/T) and the community
- Knowledge of state and federal regulations.
- Transportation: Reliable transportation and valid auto liability insurance.
Physical Requirements
- Physical and Mental Effort: Prolonged standing and walking required, with ability to lift up to 50lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psychosocial needs. Requires eye-hand coordination and manual dexterity.
- Environmental and Working Conditions: Works in patient homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
- Ability to communicate with patients, families, physicians, co-workers, and visitors to be able to exchange accurate information regarding patient condition and health status. Ability to exchange and express information by means of language and communicate information effectively.
- Ability to hear the nature of sounds. Ability to give full attention to what other people are saying, take time to understand the points being made, ask questions as appropriate, and not interrupt at inappropriate times.
- Ability to make independent judgments and decisions. Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
- Ability to view tablet, Electronic Health Record, and review, assess, record or type data quickly and accurately.
- Ability to make independent judgments and decisions is required.
- Ability to determine resources needed to provide quality patient care.
- Ability to travel to patient homes and office/support center locations as needed to deliver care or for education purposes.
