Driver Files and Eligibility
Driver files:
ยทย  ย  ย  ย For new drivers:
o ย  Make sure driver completes all necessary paperwork
o ย  Follow FMCSA guidelines to contact previous employer regarding driving record and testing results
o ย  Copy of driver record from state agency
o ย  Send driver for initial drug screen
o ย  Review DMP rules, code of conduct and drug policy โ€“ have them sign acknowledgement
o ย  Obtain copy of medical card and driverโ€™s license
o ย  Prior to first drive, send for alcohol screen
o ย  DMP driver agreements and substance abuse policy
ย 
ยทย  ย  ย  ย For current drivers:
o ย  Annual declaration of violations (to be completed by driver)
o ย  Obtain a copy of driving record from state agency (every year)
o ย  Send drivers for randomly selected drug and alcohol screens
ย 
ยทย  ย  ย  ย For terminated drivers:
o ย  Document files with termination paperwork
o ย  Remove them from testing pool
ย 
Other eligibility requirements
ยทย  ย  ย  ย Maintain a calendar of expiration dates for current driver medical cards and licenses
ยทย  ย  ย  ย Maintain records of random drug and alcohol testing, as well as results
ย 
Insurance
ยทย  ย  ย  ย Communicate with Mary regarding drivers to be added or removed from DMP insurance (secure appropriate info from drivers if needed)

Driver Training
