Job description

Responsible for directing strategy and operations for population health programs, to improve the health of SBH patients and our community. Provides strategic direction, leadership and vision to program teams, overseeing daily operations and management. Supports the Vice President to improve care quality, strengthen relationships and grow business.

  • Directs strategy and daily management of assigned program operations and workflows.
  • Directly supervises and manages assigned program staff on a day-to-day basis, interviews and hires, evaluates and documents staff performance and appraisals.
  • Manages all administrative functions, coordinating daily operations, and communicating changes to workflows, and policies and procedures
  • Leads continuous improvement of assigned program processes and patient experience. Responsible for planning, directing, communicating, and executing continuous quality and process improvement plans.
  • Monitors effectiveness and efficiency of processes and outcomes, and reports data to the Vice President.
  • Assists the Vice President in developing and maintaining relationships with internal and external stakeholders.
  • Assigned programs to include Pathfinder Program, providing 7-day follow-up from ED and inpatient units for patients with a diagnosis of alcohol, drugs, or serious mental illness.
  • Assigned programs to include Medication Adherence and Medicaid Recertification program.
  • Assigned programs to include quality improvement initiatives for meaningful patient encounters and patient experience.
  • Assists leadership in the strategic planning and implementation of the EPIC electronic health record, ensuring the department, team and referring providers are ready for go-live.
  • Prepares reports and tracks internal processes, presenting findings to SBH leadership and key stakeholders.
  1. Education: Master's degree in Health, Business Administration, or other related field preferred. Bachelor's degree in Health, Business Administration, or other related field essential.
