Job description

DIRECTOR OF POPULATION HEALTH (QUALITY)
Lincoln Community Health Center, Inc., a federally qualified community health center located in Durham, NC is recruiting for a Director of Population Health. Durham is known as the Bull City, the City of Medicine, the Tastiest Town in the South, and the Startup Capital of the South. It's a two-hour drive from the beach, a three-hour drive from the Blue Ridge Mountains, and is North Carolina's fourth largest city by population.
OCCUPATIONAL SUMMARY:
The Director of Population Health is responsible for the oversight, leadership and delivery of population health, care management and care coordination programs. Provides leadership, direction and strategic vision for population health in collaboration with the Chief Medical Officer and Senior Leadership. Works collaboratively with Senior Leadership, Clinical and Administrative Staff to design, develop and implement programs resulting in measurable improvement in population health outcomes.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Comprehensive understanding of population health, best practices, programmatic implementation and monitoring of key population health measures. Ability to motivate and engage staff and clinicians, build trust and consensus, and appropriately lead, delegate and empower a high-caliber team. Strategic thinker with strong communication skills and initiative who critically analyzes problems and generates creative and effective solutions and can work concurrently on a variety of projects with competing deadlines and turnaround times. Caring and compassionate with a commitment to working in community health with the challenges of working with an uninsured and diverse patient population.
EDUCATION REQUIRED:
Master's degree or Advance Practice Provider with Management Experience preferred; Bachelor's degree required.
PREVIOUS EXPERIENCE REQUIRED:
A minimum of three to five years management experience with more than 3 years of demonstrated care management/population health experience preferably within a Case Management Department.
