Home Health Registered Nurse

What You Will Do
As an RN Case Manager, you will provide skilled nursing care in the home while guiding each patient's plan of care from start to finish.
  • Assess, plan, implement, and evaluate individualized patient care
  • Develop care plans in collaboration with physicians and the interdisciplinary team
  • Complete admissions and ongoing nursing assessments
  • Educate and support patients and families through changing needs
  • Document care accurately and in a timely manner

Salary Range: $72,000 to $90,000 annually, based on experience
