Digital Care RN Coordinator (7p-7a)

Description

Summary: This RN position will assess, plan, implement and evaluate care processes for digitally enabled strategies across Ballad. The RN Coordinator will lead the virtual health clinical initiatives with our digital care providers along with ensure the best possible experience for our patients. This position will facilitate and assist in the care plans for all digital care initiatives that require care coordination and be the clinical liaison between the providers and the digital care implementation team. He/she will also assist in care coordination of digital care patients using standard clinical processes. This position will communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all digital care patients. This position requires the ability to effect change, perform critical analysis, promote client/family autonomy and plan and organize effectively across the continuum of care. This position will lead the clinical care and use critical thinking skills to evaluate and care for the patient. This position will facilitate training sessions with our clinical team as needed along with being responsible for creating and maintaining all clinical education material for digital care. Minimum 8 years clinical experience in ICU, ED or home care setting.
