Job description

PURPOSE OF POSITION:

The purpose of the Decentralized Pharmacist position is to review medication orders, provide clinical drug information, and provide pharmacy support as a nursing unit-based pharmacist.  Additionally, the Decentralized Pharmacist will provide all regulatory review of medication distribution on covered nursing units.

RESPONSIBILITIES & EXPECTATIONS:

Decentralized Pharmacist Responsibilities:

  • Reviews, interprets, and verifies all physician orders for processing.
  • Provides safe, efficient, patient-centered, evidence-based clinical pharmacy services including pharmacokinetic consultation, drug therapy monitoring, healthcare staff in-services, patient-specific consultations.
  • Provides cost-effective, evidence-based drug information to providers and nursing staff.
  • Performs final check/verification of all processed orders and approves for delivery and release.
  • Clarifies medications, doses, routes of administration and other related information with the physician as indicated.
  • Knowledgeable of age specific doses and routes of administration.
  • Knowledgeable of drug/drug interaction, food/drug interaction, renal clearance dosing and adjustments, indications and contraindications, compatibility, and stability of intravenous medications. That includes chemotherapy medications and Total Parenteral Nutrition.
  • Prepares and/or supervises the preparation of extemporaneously compounded medications, including sterile products.
  • Inputs patient and medication order information accurately into hospital computer system via direct order entry or CPOE verification to facilitate medication dispensing.
  • Provides concise drug information to physicians, nurses, patients, and other allied health professionals.
  • Knowledgeable of different drug information references and sources including websites.
  • Maintains control and security of all medications and reports all potential diversion to the Director of Pharmacy.
  • Supervises the activities of pharmacy support personnel related to the preparation and processing of orders and their performance.
  • Communicates with physicians, nurses, patients, and other allied health professionals in a courteous, professional and customer friendly manner.
  •   Performs correct calculations of doses and infusion rates.
  •   Participates in clinical activities including vancomycin dosing, anticoagulation monitoring, and any other clinical programs assigned by the department.
  •   Completes licensure continuing education requirements and work-related competencies in a timely manner.
  • Understands regulatory requirements and standards affecting his/her scope of practice.

· Follows medication use policies and complies with drug formulary policies and procedures, including enforcement of the Pharmacy and Therapeutics Committee approved therapeutic interchanges and non-formulary requests.

· Provides education to providers when they have problems/issues entering medication orders into the Cerner Millennium information system.

· Enters medication variances in Midas & enters clinical interventions identified in Sentri7 and by self into Quantifi software (or other current intervention tracking software used by Aultman Pharmacy.)

· Assists in the training and development of new pharmacy employees.

· Assists in the training and education of pharmacy students and pharmacy residents.

  • Performs other duties as assigned by the supervisor.

JOB REQUIREMENTS:


 
  • Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy degree (PharmD) from a college or a university accredited by the American Association of Colleges of Pharmacy.
  • Licensure/Certification/Registration: Pharmacy License (RPh) in the State of Ohio, Board Certification.
  • Completion of ASHP-Accredited Post-Graduate Year 1 (PGY-1) pharmacy residency or 3 years of relevant post-graduate hospital experience with clinical responsibilities.
  • Certification in Advanced Cardiovascular Life Support (ACLS) and Basic Life Support (BLS) through American Heart Association (AHA).
  • Board certification with the Board of Pharmacy Specialties (BPS) in Pharmacotherapy or other specialty areas preferred.
