Key ResponsibilitiesProgram & Service Coordination
  • Coordinate and oversee services for individuals enrolled in NJ DDD programs.

  • Conduct intake assessments and complete all required intake documentation.

  • Develop and implement individualized service plans based on each person's needs, goals, and preferences.

  • Ensure participants receive quality, person-centered supports that promote independence and community inclusion.

Community-Based Services
  • Support individuals in accessing community activities, appointments, employment opportunities, and social engagement.

  • Coordinate referrals and connect individuals with appropriate resources and service providers.

  • Maintain strong relationships with families, support coordinators, community partners, and external agencies.

Leadership & Staff Supervision
  • Supervise and support direct care staff and program personnel.

Compliance & Administration
