Location: Springfield, MA

Program: Individual Supports

Hours: FT 40hr/week (20hrs Case Manager + 20hrs DSP)

Pay Range: $24 – $24 per hour

Responsibilities

Responsibilities: The qualified candidate must be able to work effectively as a team member, be resourceful with excellent communication skills and have a measure of flexibility to ensure they are able to help meet the needs and mitigate the challenges in the lives of the individuals we support.

20 hours case management, 20 hours direct care, converting to full-time case management over time, which will be discussed during the interview.

• Assist individuals to identify and address their needs.

• Assist individuals with activities of independent living such as money management, household management, medication management, health maintenance- medical, dental and clinical appointments and follow-up.

• Assist individuals to access and maintain public benefits including but not limited to food stamps, Mass Health, Medicare, Social security, housing, fuel assistance.

• Assist individuals with making community connections to maximize their quality of life such as Best Buddies, Special Olympics, Senior center, YMCA etc.

• Supervise and work with direct care staff

• Work with DDS as well as various social service agencies such as DCF, MHA, BHN etc.

Required Qualifications, Skills, and Training
    • Associate Degree in Human Services or other related field preferred.
    • Three years of recent experience working with individuals with Intellectual Disabilities.
