Home Health Scheduler
Fast-Paced, High-Impact Scheduling Role
At Elara Caring, delivering the right care, at the right time, in the right place starts with ensuring every patient has the right clinician scheduled when they need them.
We are seeking an experienced Home Health Scheduler who understands the pace, urgency, and complexity of coordinating care in a busy home health environment. This is not simply an appointment-setting position-you will serve as the central connection between patients, field clinicians, clinical leadership, physician orders, and operational needs.
Schedules can change quickly, patient needs are time-sensitive, and multiple priorities often require attention at once. To be successful, you must be highly organized, comfortable making real-time adjustments, and able to communicate clearly while keeping patient care moving forward. Previous home health experience is required because understanding visit types, clinical disciplines, physician orders, compliance requirements, and field staffing is essential to succeeding in this role.

What You'll Do
As a Home Health Clinical Scheduler, you will:

  • Coordinate and maintain busy daily and weekly schedules for RNs, LPNs, and other clinical team members.
  • Schedule initial evaluations, start-of-care visits, transfers, reassessments, recertifications, supervisory visits, and routine clinical visits.
  • Ensure visits are scheduled according to physician orders, patient needs, clinician availability, geographic territory, and regulatory requirements.
  • Monitor schedules throughout the day and respond quickly to call-outs, missed visits, changing patient needs, hospitalizations, and other unexpected updates.
  • Partner closely with the Director of Nursing, Clinical Team Manager, Case Managers, and field clinicians to identify staffing needs and resolve coverage gaps.
  • Communicate schedule changes promptly and professionally to patients, caregivers, clinicians, referral sources, and other members of the care team.
  • Review clinical systems to monitor completed evaluations, documentation, and messages to Case Managers.
  • Confirm visits are entered accurately and timely to support clinical compliance and billing.
  • Monitor Electronic Visit Verification (EVV) activity and notify clinical leadership of missing or noncompliant usage.
  • Assist with nursing delivery reports, schedule audits, staffing reports, and other operational tracking.
  • Protect patient and employee privacy while maintaining full compliance with HIPAA requirements.
  • Provide exceptional service-even when priorities shift, the phones are busy, and the schedule requires creative problem-solving.
  • Work collaboratively with team members to address concerns, remove barriers, and ensure continuity of patient care.
  • Provide additional administrative and clinical-team support as needed.
What You'll Bring
  • High school diploma or GED required.
  • Previous home health scheduling or home health operations experience required.
  • Working knowledge of home health visit types, clinical disciplines, physician orders, scheduling requirements, and field-based care.
  • Experience working with electronic medical record, scheduling, or healthcare-specific software; Homecare Homebase experience is a plus.
  • Ability to manage a high volume of calls, schedule changes, and competing priorities with accuracy and urgency.
  • Strong organizational, time-management, and problem-solving skills.
  • Excellent written and verbal communication skills.
  • A calm, professional approach when handling urgent needs or difficult conversations.
  • Strong attention to detail and comfort working with reports, data entry, and multiple computer systems.
  • Reliable transportation and current vehicle registration, as required to perform job responsibilities.
