Job description

POSITION SUMMARY: To provide direct support to participants with intellectual and developmental disabilities to promote their personal and professional growth in both employment, community, and day program settings.

QUALIFICATION: A high school diploma or equivalent. Experience working with individuals with disabilities are preferred but not required.

ESSENTIAL DUTIES:


A. AGENCY

1. Performs essential job duties with or without reasonable accommodations.

2. Knows and complies with all required policies, procedures, regulations, and guidelines of Target and has a clear understanding of the specific regulations used in their department. Follows regulations and guidelines when carrying out duties and responsibilities.

3. Successfully completes all required in-service trainings and completes re-certifications as require; demonstrates a clear understanding of the issues presented in trainings; is eager to expand knowledge; keeps current with relevant changes and take appropriate steps obtain needed information. DDA trainings must be completed within the first 90 days of employment.

4. Performs duties in accordance with the philosophy, mission, and goals of the agency.

5. Works as a professional collaborative with all internal and external stakeholders in modeling a safe, supportive, and respectful demeanor ensuring the highest quality services to all Target participants.

6. Takes appropriate action based on circumstances; provides pertinent and comprehensive information to supervisory personnel.

7. Demonstrates the ability to manage personal time by being punctual and gives sufficient notice to immediate supervisor in case if tardiness or absenteeism per Target#39;s Attendance Policy.

8. Acts as an appropriate role model and representative of Target as an agency as well as those employed by Target and those served. Choose appropriate personal attire base on the circumstances. Maintains appropriate overall appearance and demonstrates proper personal hygiene per Target#39;s Attire and Grooming Policy.

9. Demonstrates a willingness to perform all essential duties and job functions with the safety of the participants and self as a top priority; adheres to and complies with all safety guidelines taught in trainings.


B. ADMINISTRATIVE:


1. Facilitate meaningful day programming by assisting with classroom-based educational activities, vocational skill building, and community integration.

2. Complete thorough and detailed service logs in CIMS portal for each participant as scheduled in a timely manner. Document all medical and behavioral data as necessary on CIMS in a timely manner.

3. Attend all scheduled meetings.


C. DIRECT CARE:

1. Provide direct care and supervision to individuals in the day program setting, job site, or out in the community.

2. Maintains physical ability to restrain participants as needed according to their behavior program plans. The position also requires the ability to stand, bend, reach, lift 50 pounds, maneuver wheelchairs, and perform safe transfers of individuals.

3. Job coach individuals at their employment or volunteer sites; help to train workplace skills and foster independence; support individuals in carrying out their duties to the standards of their employers and/or supervisors.

4. Implement behavior management plans and collect data accordingly.

5. Support participants in achieving their vocational goals, as outlined in their Person-Centered Plans (PCPs); collect goal data accordingly.

6. Provide safe transportation for participants to and from their homes, employment sites, community destinations, and the day program, if applicable, in company vehicles. Fill gas tanks in vehicles as needed, complete regular vehicle inspections. Mileage reimbursement may be available for use of personal vehicles but requires proof of proper current insurance, kept with HR.

7. A Meaningful Day DSP may be required to maintain an active Medication Technician Certification depending on the setting or participant assigned.


OTHER RESPONSABILITIES: Other duties as assigned by Supervisors.


SUPERVISON: The position is supervised by a Meaningful Day Manager and the Director.


EVALUATION: After an initial three-month period, then annually during term of employment.


TERMS: This is a Class 2, part-time or full-time, hourly, non-except position. Scheduled hours may fluctuate to meet the needs of the participants served. Weekends, evenings, and holidays may be required. This position is considered quot;essentialquot; to maintain operations and services to participants. I understand that while employed by Target Community amp; Educational Services, Inc., I will not use a company vehicle for personal use.
